About the Role

We are seeking a compassionate, organised, and proactive Transition Coordinator to join the Enliven team. This role is pivotal in guiding future tenants through their transition into Enliven Housing, ensuring they feel supported every step of the way.

As part of our collaborative and hands-on team, you will provide personalised support, help navigate the complexities of securing appropriate housing, and engage with tenants, their support networks, and service providers to ensure a smooth transition.

Key Responsibilities

• Tenant Support - Assess tenancy applications, engage with successful applicants, and provide transition planning.
• Engage & Educate - Present Enliven Housing’s model to the sector through webinars, events, expos, and forums.
• Stakeholder Collaboration - Work closely with Onsite Support Providers, Internal teams, and external networks to ensure seamless integration for tenants.
• Property Engagement - Conduct virtual and in-person property viewings and facilitate key meetings.
• Customer Service - Act as a primary point of contact, providing outstanding support to tenants and their support networks.
  
  About You
  
  
• A passion for making a difference in the disability sector.
• Excellent communication and relationship-building skills.
• Year 12 Certificate of Education (or equivalent).
• Undergraduate degree in Social Work or Business (highly desirable).
• A problem-solving mindset with the ability to navigate challenges.
• Highly organised, with strong attention to detail and the ability to manage multiple tasks.
• Experience in disability, community housing, or social work (desirable).
• Proficiency in Microsoft Office and Adobe.
• A valid NDIS Worker Check (or willingness to obtain).
• Ability to travel to Enliven Housing properties and attend industry events.
