Katungul Aboriginal Corporation Regional Health and Community Services works in partnership with local health services to ensure that their clients receive culturally appropriate medical, dental, social and emotional wellbeing services. Their vision is "for Aboriginal people to live healthy lives enriched by a strong living culture, dignity and justice and for Katungul's work to be recognised as a driving force in achieving this.We have 3-month temporary assignments, with the opportunity to go permanent, for an NDIS Support Coordinator who will be responsible for establishing a positive collaborative relationship with individual clients and their support network. You will assist the participants to identify, link with and coordinate support to link with local communities, build skills, overcome barriers and achieve goals. The Coordinator will have the responsibility to ensure the implementation of the person's plans and the achievement of their goals.Ensure participant supports are managed within the budget parameters of their NDIS plansProvide support and direction to support staff as directed by immediate supervisorLiaise with support staff regarding shift and rosters for participantsMaintain complete case notes and other records and maintain accurate recordsEnsure workflow documents are up to date and reflect the correct procedures and processesThe ideal person will have experience in an Aboriginal health/ disability organisation and qualifications in health administration or a related discipline. You will have strong IT skills and have experience working with external health service providers and other community-based services and organisations.You must also have a driver's licence, police check, working with children check and be fully vaccinated against COVID-19.This full-time role is remunerated under the Social Community, Home Care and Disability Services Industry Award 2010 [MA000100]Casual rate: $40.68/ hourPermanent rate: $32.54/ hour Position Description AUD Australia - NSW Wollongong, Illawarra & South Coast Narooma 2546
